Our Team
Our VH-CCYP team of caring professionals are here to serve you.
- At VH-CCYP your child and family will be assigned an Assessment Coordinator who is a Behavioral Health Clinician and will serve as care manager for your needs. They will provide a clinical assessment, work with you on a care plan, ensure that you have access to all the resources that you need, help you measure progress, and be the primary point of contact to keep you and your child connected to care.
- The team at VH-CCYP also includes a board-certified child and adolescent Psychiatrist who will provide a psychiatric evaluation and discuss with you all options for medication treatment to develop the best plan. The Psychiatrist works closely with the Assessment Coordinator, youth, and family to ensure the best outcomes and easy access to care.
- When requested by the Assessment Coordinator and family, a Case Manager is assigned to work with youth and families to help them connect to community resources. Case Managers will work with you to identify needs, provide a warm handoff to the people providing that resource, and will follow-through to make sure the resource meets your needs.
- Our Registered Nurse, works closely with your child’s primary care provider to coordinate all medical care issues and, as appropriate, develop a plan for your primary care team to eventually take on the medication management needs.
- The VH-CCYP team is led by a Licensed Program Manager, who is responsible for all program operations and the clinical work of the staff.
